To make your experience at Body Wisdom Therapeutics as seamless as possible I would like to offer a few suggestions.
First, download a "First-Time Client Health History Form" from the "Client Forms" page, fill it out and bring it with you to your appointment. If you don't have access to a printer I have plently of copies at my office.
Second, please attempt to arrive a few minutes before your scheduled appointment time as this will allow us the opportunity to discuss your needs and medical issues prior to receiving the massage.
Third, if you schedule an appointment and need to cancel or reschedule please let me know at least 24 hours in advance. Otherwise, I will be in the unfortunate position of having to charge you for your missed appointment.
Also, if you are going to be late please call and let me know. If you do not call and do not show up within 15 minutes of your appointed time you will be considered a "no show" and will be charged accordingly by forfeiting your Groupon. Typically I will wait for 15 or 20 minutes before I consider the treatment time a "loss" and will leave to conduct other business before my next client arrives.
The Groupon applies to office visits only.
